We assist in identifying community resources for funding and potential support available to our clients, including the non-means tested NHS continuing healthcare funding, commonly referred to as CHC.
The CHC funding process may be initiated following an assessment, and we can guide you through the referral process while working closely with the CHC Nurse teams, with whom we have built strong professional relationships, ensuring you remain informed throughout.
